COLON TISSUE MICROARRAY AGREEMENTColon TMAs are provided as a service to the research community without warranty of merchantability or fitness for a particular purpose or any other warranty, express or implied. The investigators must agree that the tissue microarrays (TMAs) provided will be used for the research purposes specified in their letter of intent and will not be transferred to any other investigator(s). The NCI is interested in learning more about marker activity in the tissue microarray specimens so that the quality/performance of the arrays can be assessed. As a condition of receiving specimens, the investigator is required to return the results from the study to the NCI. This data is for the NCI internal use only. The confidentiality of the data will be maintained and will not be shared with anyone else and the investigator's right to publish results will not be restricted. The investigator also agrees to acknowledge the contributions of the NCI in all publications resulting from your use of Colon TMA slides. Recommended wording to the methods or acknowledgment section is as follows: Tissue microarray slides were provided by the National Cancer Institute. Other investigators may have received slides from these same array blocks. Please provide the NCI with the citation of any publication resulting from research on Colon TMA slides as soon as it is available. THE APPROVED SPECIMENS WILL BE PROVIDED UPON RECEIPT OF THIS SIGNED FORM. Please email an acknowledgement of this document to tissueresources@imsweb.com, and fax the signed form to Cindy Mattingly at Information Management Services at (301) 680-8304. We will be happy to discuss these requirements and the timing of your submission with you prior to shipment of the specimens.
